Order Starter Kit
Your starter kit includes two Rx forms, our fee schedule, case box and 2 pre-paid shipping labels. Once you have established an account with CMC you will receive special discounted pricing for U.P.S. overnight shipping. The nominal fee of $12 will be billed to your account and you can order the labels from our website at www.custom-milling.com/discountshipping.
What to include with your first case:
- Completed Rx form
- Models and/or Impressions (To ensure functional occlusion your model must be mounted on a plastic disposable articulator)
- Study and/or pre-op models (if available)
- Patient photos
- Bite registration
- Shade guide (if necessary)
